Understand the social determinants shaping behavioral health in your community

County-level SDOH data across seven domains aligned with CCBHC 2025 screening requirements. Identify risk factors, compare to state and national benchmarks, and support your community needs assessment.

Data Sources & Methodology

Data Sources

  • County Health Rankings & Roadmaps (2024) — University of Wisconsin Population Health Institute
  • U.S. Census Bureau American Community Survey (ACS) 5-Year Estimates
  • CDC PLACES: Local Data for Better Health
  • USDA Food Environment Atlas

Methodology

Domain risk scores are calculated using z-score normalization of underlying measures, converted to a 0-100 scale via cumulative distribution function (CDF) mapping. Higher scores indicate greater risk relative to the national distribution. Composite risk is the unweighted mean of all seven domain scores.
